If further details are needed, or the service request is invalid, the approver can deny the form submission or send back to the submitter for more details. Once approved, data from the form is routed into a work order document and sent to the maintenance team responsible for the identified school, dorm, or site. 

Final Sign-Off 

Upon completion of the work, the maintenance team member can sign off on the work order. Formstack Sign’s multiple participant workflow will then automatically send the work order document to the service requestor for eSignature to confirm completion of the request.
